Arvin B. Casimiro filed an affidavit of loss stating that he is the registered owner of a 2018 Suzuki FU150CD3 motorcycle. He was issued a certificate of registration and official receipt by the Land Transportation Office in Zamboanga City. However, in August 2020 he discovered that the certificate of registration and official receipt had been lost and could not be located despite diligent efforts to find them. Through this affidavit, he is declaring the documents lost and beyond recovery to comply with Land Transportation Office requirements in lieu of the lost documents.

AFFIDAVIT OF LOSS

I, ARVIN B. CASIMIRO, of legal age, Filipino citizen, and a resident of Zone 6,


Talabaan, Zamboanga City, Philippines, after having been duly sworn to in accordance with law,
hereby depose and say, that:

1. I am the registered owner of that certain motor vehicle duly registered with the Land
Transportation Office, which is more particularly described as follows:

MAKE : SUZUKI
SERIES : FU150CD3
YEAR MODEL : 2018
ENGINE NO. : G427-318429
CHASSIS NO. : BG41K-212934

as such, I was issued a Certificate of Registration (CR for brevity) bearing number
307096801 and an Official Receipt (OR for brevity) bearing number 1694428794 by
the Land Transportation Office, Zamboanga City (LTO for brevity);

2. The above-described motor-vehicle registered under said CR is not mortgaged/used


as collateral to any individual and/or any financing institution;

3. However, last August 2020, I discovered that said CR and OR got lost and cannot be
located;

4. I exerted diligent efforts to look and locate for the above-mentioned CR and OR in
other places, but all such efforts proved futile and to no avail;

5. For all intents and purposes, I am now declaring the above-mentioned CR and OR
lost and beyond recovery;

6. I make these statements under oath and in good faith in order to establish the fact of
loss of the above-mentioned CR and OR;

7. I execute this Affidavit to attest to the truth of the foregoing facts based on my
personal knowledge and authentic records in compliance with the requirement of the
LTO in lieu of the lost ones and for whatever legal purposes it may serve.

IN WITNESS WHEREOF, I have hereunto set my hand on this 9 h day of September
