AFFIDAVIT
I, Varri Sureedamma w/o late Mahesh, Hindu, aged about 45 years, labourer, residing
at Door No; 1-1, Yeguva Veedhi, Neelavthi Village, Gantyada Mandal, Vizianagaram district, do
hereby solemnly affirm and state on oath as follows:-
I hereby declare that my husband’s name is Varri Mahesh who died on 13-12-2015 and I
am doing labour work when it is available. I further declare that I have no source of income
what so ever except from my labour work. I further declare that I belong to Koppala Velam
caste which is classified as B.C-D. I further declare that I am not getting any benefit under
welfare schemes from the Govt. A.P. and as such I am an eligible person to get monthly pension
under the scheme of Y.S.R Widow pension scheme. My Aadhar card No: 6277 1750 4879. I am
affirmed the contents of this affidavit to submit the same to the concerned Authorities. Hence
this affidavit.
