PART I: HEALTH CARE DELIVERY SYSTEM

Before discussing the significance and importance of CBR, it is crucial to know the primary foundation, the
health care delivery system. If you just remember your health care subject, you’ve already discussed the health
care system and its contextual differences. Synonymously, the health care system and CBR program are not too
different things. One has to work appropriately to make the other becomes effective and efficient towards its target
stakeholders.

But what do we mean when we say health care delivery system? What is it for? If you ask your friend, brother, sister, or
neighbor, most of them might answer this way, “It is providing health to every individual,” or maybe others will tell you, “It is
more of getting appropriate health services from the government.” Well, at some point, they are correct.
But if I ask you the same questions, how will you answer them base on your fundamental ideas?

I provide you a table below where you can place your answers. Using the text box, I want you to answer these questions:

Primarily, the concept of Community-based Rehabilitation (CBR) is the same as how you adopt and implement the healthcare
delivery system's principles. Nevertheless, if we look at them differently, you'll appreciate that these two strategies work simultaneously
and can have common goals for the community. Few things that make them different are the target population. Community health
development deals with the health of each member of the community no matter what kind of group of people they belong to or perhaps
looking at the health status of the community as a whole in general. On the other hand, the CBR program is designed to cater to the
need for a specialized group of people in the community, People with disabilities, including their families. Remember that the basic
premise of both health care and CBR is illness prevention and health promotion.
But anyway, you'll find in the middle part of this comprehensive training module the other unique characteristics of each
program that we are talking about. In the meantime, let's just focus on the health care system.

Community Health Centers

Nowadays, it is essential in the community to have facilities that can accommodate several people seeking medical attention,
whether for health maintenance or seeking medical treatment. In the local community, the health center should be well-represented as
the health facility where an individual can acquire immediate medical attention. However, for some reason, not all village and
municipality have their health facility that can accommodate the influx of people that are seeking medical attention, whether it is simple
or complex health cases. For some instance, even in the presence of health care structures and facilities, its capacity to handle such
cases remain an issue due to lack of sufficient medical equipment and materials, workforce issue or inappropriate allocation of
resources that might be helpful to address or at least lessen the effect of illness or disease among individual will medical issue.

The purpose of this topic is for you to understand and realize the importance of the presence of health care centers or facilities
in the local community. This will be just simple information that most of us often forget.

For your next activity, I want you to grab your references textbook and try to answer the table below. A short video clip

about and discuss the different levels of the clientele of health care. The importance of this topic is to figure out who can benefit from
CHC services and how it will affect the delivery of medical services to all people who need them.

As simple as this! In the CBR program, we advocate, and we implement! And because we implement, we focus on addressing
some issues and concerns that are relevant to the welfare of the identified specialized population in the community and
PEOPLE WITH DISABILITIES.

Finally, you’re done to the level of clientele. I commended you for your
perseverance and diligence to answer this table. Well, I think this topic was not
thoroughly discussed in your health care subject. Still, you’ll appreciate this with
this subject, for it may eventually affect your CBR program implementation at the
later part of this program.

Wait, have you notice, we always keep on mentioning “community” from time to time.
But what do we mean when we say community? How do we define community in the context of health and CBR? Who do we consider
as part of the community? How does the community affect our lives and vice versa?

We can define community in different ways depending on our view of point. By definition, a community is “A group of
people living in a particular area or place.” Community is not limited to just talking about the particular group with the same
norms, religious beliefs, races, cultures, etc. Community is more than the geographical structure with boundaries where everyone lives
or even more than its topographical identity. Basically, the community can be defined depending on what you want to know about it.
We can define community in the context of health, its political involvement and perspective, community and its people, or perhaps,
describe it in the context of socio-economical domains. But again, what we are trying to extract as of this moment is how we can
correlate the definition of community in the context of health and eventually its implication to the principles of the CBR Program.

The Health Care Delivery System

At this point, I am confident that you already have the basic idea about the desire and intention of health care delivery systems
towards people. Now that you know what the health care delivery system is all about let's see how a person should receive and
acquires medical services through the health care delivery system and how it can benefit the entire community.
Many techniques and strategies can be used to ensure the orderly and proper delivery of each individual's health care
services. However, some factors have to be considered before saying that the health care system is effectively delivered to all
stakeholders. For example, Person 1 will need to see a specialist to assess and determine his cough cause. Unfortunately, person one
lives in a remote area where he always finds it hard to reach the nearest community health center. On the other side, person two also
experiences the same problems as person one and seeks medical services, particularly treatment procedures. But the difference
between them is person two live in a wealthy village where he can quickly get medical attention because of the proximity to the nearest
community health center. If we use the health care delivery system's principles, who do you think between these two persons has the
point of disadvantage in acquiring medical services and treatment? Which of them will eventually be cured? And who would not be?
Probably you will think it's the person two has much likely be on the positive side. Why? Because of the proximity of the health care
facilities and perhaps, he lives in urban cities. There are good and decent medical facilities where he can find a medical specialist to
treat his illness. Unlike the person one, it requires him to put extra effort into resolving his medical problems. Although these two
individuals live in different states in life, their medical issues can be resolved differently. Both scenarios about how competent and well-
organize the health care delivery system in their respective places.
Health care system! Maybe for some of you, this is not something new. The word itself implies a systematic way to deliver
holistically and organize health care services to each and everyone in society.

For your next activity, I would like to see how well you know the health care system. Try not to complicate everything. I want a
practical and straightforward definition of it base on your knowledge.

PART II: THE PHILIPPINE HEALTH CARE SYSTEM

They said, “We are responsible for our wellness.” It’s true! In our country, many laws have been created to ensure
fair and appropriate measures to promote equal opportunities in receiving medical services. But on the other side, despite all of those
laws and legislation, there are still people from the community who haven’t experienced these decent medical services offered by
the government.
One possible reason is the inadequate programs and policies that strongly implement that help the health authorities at the local
communities ensure positive health status among all community members.
Now, let’s talk about the common health-related laws and legislation that are primarily designed for appropriate, fair, and
suitable health services for all Filipino citizens. Don’t worry, and I will not ask you to describe or explain all listed laws in the table
below. This will be a piece of giveaway information, and all you have to do is read and be familiarized with all common health law.

This time, plenty of information is now circulating in your head regarding the purpose of the DOH. Before, we simply know
them as the government agency responsible for delivering health services in our country. Yes, it may be right, and if it is, that is
something suitable for all of us. But in addition to this, DOH systematically works with other government agencies to give the most
appropriate and applicable health care system. E.g., concerning PWD, the DOH should work closely with the National Council for
Disability Affairs (NCDA), Department of Social Welfare Development (DSWD), and Local Government Units (LGUs). DOH also works
with the Department of Science and Technology (DOST) to provide advancement and innovative medical technologies, equipment and
modalities and so on. So, it means that this agency is not working alone. Failure to collaborate with the different agencies will suffer the
system in which the people mostly be affected.

PART III: LEVEL OF HEALTH CARE FACILITIES

This is just a preview for you about the three levels of facilities. I‘m pretty sure that you have what it takes to enumerate
these three levels of health care facilitates and differentiate them from one another. So, the table below will allow you to review these

It is required for you to understand the differences between these three levels of healthcare facilities.
However, we are trying to focus on the concept of Primary health care (PHC). Later on, in the discussion, you will
find out the relevance of PHC to the concept of CBR.

Primary Health Care (PHC.)

Going further to the most in-depth detail about this chapter, I would say that this is essential. Yes, you heard it right! The
reason why I wanted you to understand the concept of PHC better because the strategies used in implementing PHC at the community
level are almost the same as how do Community-based Rehabilitation (CBR) is being implemented at the community level. Again, as I
mentioned in the early part of this chapter, it only varies on its target population. In PHC, it targets all individual in the community,
Gender, age, socio-economic status, etc., all of them are essential in the implementation of PHC. In other words, every person in the
community has a vital role in implementing and adapting PHC. All aspects are involved and make sure that they contribute to the
development of health programs at this level. As mentioned above, the primary focus of PHC always stars from the promotion of health
and wellness and prevention of illness if just you notice, PHC focus on the health component alone.

Whereas, in CBR, we only focus on the identified specialized population: People with Disabilities (P.W.D.), including their
families. But wait, don’t get me wrong. That does not mean that other people in the community, a person without disabilities, will
have no contribution to CBR implementation. It’s just that PWD are the main target of the CBR program. It is also essential to
empower the whole community to have an inclusive and accommodating environment for all PWD and their families. Soon, as we go
through the succeeding chapters of this comprehensive worksheet and module, you’ll get to know better about the concept of
CBR and how it works towards PWD and as well as to the community in general.

Going back to PHC, the table below will allow you to see the different PHC implementation concepts. I want you to grab your reference
textbooks together with your lecture materials and try to complete the activity provided below. This activity will allow you to explore the
basic concept, idea, and information about what PHC is all about.

Thomas, I will not ask you any more details about the similarities and differences between PCH and CBR. What is important for now
is for you to deeply understand the principles beyond the concept of applying and implementing PHC. Just master the table from

Activity #: 1-11 Health Care Delivery System: A Simple Analysis

Description:
Imagine that the health care delivery system is like a river with strong water flowing down the stream. At the other side of the river, a small bridge
that is already not in good condition made from scrap wood in which people in the village used to cross every time they want to go to the other
side of the town. The bridge is quite old, and obviously, no one from the city can fix or renovate it.
One day, a group of people trying to cross the river using this bridge. Suddenly, the bridge collapsed, and people fell and were washed
away by the water's strong current flowing on the river. People were saved as expected, and no one was severely hurt.
What is the connection of this illustration in the Health care delivery system?
Cues:
Bridge = Cause of health problem
People = Individual who are desiring to have a decent health care
River = The effect of health care problem
Rescuer = Health care delivery system

As clearly seen in the illustrations, the victims/people at harm were saved because of being ready, and I think that in the world of
health care system, it is related in a way because even if there are disabled or vulnerable enough to cause some problems, everyone
would be saved if the system is ready and accessible. Facing different problems everyday is normal for all of us and it is all in the
matter of being aware and ready to help and solve it. Just like in the illustration, the bridge may be a problem that is long overdue, the
people knew already that there is a problem but they did not do anything about it to prevent the greater problem and I think that is
sometimes the problem here in the Philippines, we tend not to look in the cause of the problem or we tend to disregard the main
problem even if we already know what may happen and it is because of the people who are sometimes taking advantage of the
health care delivery system that we have right now.

Even the wealthiest countries also have problems in managing their health care system. That is why they invest and allocate
too much budget to ensure the wellness of their local citizens. But, let’s not forget how our government works hard to
deliver the best possible health care services and address health concerns and issues that every individual is facing.

For your last Activity, I want you to see the current health care situation we have right now. Let’s all admit that we have a
problem in terms of the healthcare delivery system. I want you to reflect as a Physical Therapy student and future professional, how can
you contribute to address these concerns
