AFFIDAVIT OF UNEMPLOYMENT

I, MYLEN TORILLA LIPAE, of legal age, married, Filipino citizen and a resident
of Sampaloc Drive, Baliwasan Chico, Zamboanga City, after having been duly sworn in
accordance with law hereby depose and say that:

1. I am the mother of name of child, an incoming grade 7 at Pilar College,


Zamboanga City;

2. I am presently unemployed and had no fixed income to support our standard of


living;

3. I do not own any real property or conjugal property or any landholding that is
registered in my name;

4. I am executing this affidavit to attest to the truth of the foregoing statements and
to serve as a supporting document for the scholarship application/s of my son,
name of child and for whatever legal purpose this may serve.
