AFFIDAVIT OF LOSS

I, Eden Grace S. Raguro, Filipino, of legal age and a resident of La Trinidad, Benguet,
after being duly sworn in accordance with law, depose and state that:
That I am a regular user of a Smart Sim card with no. of 09298714628 since 2019;
That on April 21, 2021, at about 11:30 to 12:30 in the morning when we were on our
way to the gym, I lost my Xiaomi Redmi note 7 cellphone where the Smart sim card is
inserted;
A few minutes after noticing that my cellphone is not with me, I returned to the
places we walked by to see if someone had it waiting for it to be returned.
I failed to recover my cellphone and the said SIM Card despite diligent search and
earnest efforts to contact and locate said phone, and I consider the same as lost and
can no longer be found;
As such, I am executing this Affidavit of Loss to attest to the truth of the foregoing
and in support of my request for the replacement of the said SIM Card and for
whatever legal purpose it may serve.
Affiant states nothing further.
IN WITNESS WHEREOF, I have hereunto affixed my signature this April day of 28,
2021 in Baguio City, Philippines.
