Re: Mr. Lionel Ramamurthy, aged 63

Mr. Ramamurthy is being discharged back into your care today. He was dignosed and
treated for Pneumonia in our hospital.

Upon his admission last February 4, he have presented with respiratory symptoms
such as shortness of breath, wheezing, as well as persistent cough that causes chest
and abdominal pain. These symptoms were accompanied by febrile episodes and
generalized body malaise.

He was weak when he came in, and assistance with ADLs were needed as it
exacerbates his shortness of breath. After a week in our care, he has improved
remarkably and is now able to ambulate and do his own hygiene independently.

Please encourage oral hydration, monitor and maintain a well balanced diet to prevent
weight loss and improve his immunity. Remind him to continue doing breathing and
coughing exercise together with postural changes to promote drainage of mucus from
the lungs.

It is important to keep him in a conducive environment to help his recovery.


Paracetamol can be given to him if he experiences chest or abdominal pain.
