Dear Hon. Gomez,

On behalf of the Multi-Stakeholders Inspectorate Team. I Joemar Dionaldo MCT-


MDE/CEF/GeoTagger of the DSWD-KALAHI-CIDSS: NCDDP LGU LA LIBERTAD. I would
like to ask permission to conduct a Sustainability Evaluation of the finished Sub-project in your
barangay last year with sub-project title: REFURBISHING OF BRGY AYA HEALTH
CENTER TO BE USED AS ISOLATION FACILITY WITH MEDICAL EQUIPMENT,
SUPPLIES AND PPEs IN RESPONSE TO COVID 19 CRISIS. The sub-project was completed
last October 10, 2020. The schedule of this activity will be on September 28, 2021 at ten (10:00)
o’clock in the morning.

I would like to invite the presence of all barangay officials, BSPMC Chairperson, and the
Operation and Maintenance Group (Representative only).
