MEDICAL EXAMINATION INSTRUCTIONS FOR


IMMIGRANT VISA APPLICANTS

Under U.S. immigration law, ALL applicants for immigrant visas are required to undergo and
complete a medical examination, regardless of age. The medical examination findings by a panel
physician are binding on the consular officer and used to determine eligibility to receive a visa.
St. Luke’s Medical Center Extension Clinic (SLMCEC) is the Embassy’s accredited medical facility.
It is located at 1177 J. Bocobo St., Ermita, Manila. The telephone numbers are 521-0020 and 521-
8647, and it is open for business Monday through Friday (except on Philippine holidays), from 7:30
a.m.-11:30 a.m. and from 1:00 p.m.-4:30 p.m. When you go to SLMCEC for medical examination,
you must bring your passport and three (3) photos (size: 2” x 2”). Prior to your visit, you are also
encouraged to register online with SLMCEC at their website: www.slec.ph. Registering online will save
processing time and make your visit to SLMCEC faster and more efficient .

It is important that you must have completed your medical examination at least a week before your
interview appointment. Otherwise, your visa application will be refused or you may be asked to
return for another appointment. The medical examination results are delivered directly to the
Embassy.

First-Come, First-Served

Applicants are examined on a first-come, first-served basis. Medical examinations may take more
than one day to complete. Female applicants may request a female doctor to perform the
examination. Unless it is necessary, do not bring companions due to space limitation at the clinic.

Fees

The medical examination fee is US$223.35 for adults (15 years of age or older) and US$185.35 for
children (14 years of age or younger). Repeat X-rays will cost US$7 each. Fees must be paid only
in Philippine pesos at the prevailing exchange rate.

X-Rays

A chest X-ray is not required for children below 15 years old, unless symptoms of tuberculosis (TB)
exist, there is a history of TB, or there has been possible exposure of TB with a known case such as a
family or household member with TB.

The U.S. Public Health Service/Centers for Disease Control (USPHS/CDC) mandates women who
are pregnant to have chest X-ray examinations conducted. Chest X-rays for pregnant women are not
dangerous if taken with the proper shielding. Applicants who have previous chest X-rays must bring
the results with them to SLMCEC for comparative analysis.
VACCINATION REQUIREMENT FOR IMMIGRANT VISA APPLICANTS

U.S. immigration law requires that ALL immigrant visa applicants be vaccinated against certain
vaccine-preventable diseases before they can be admitted to the United States for permanent
residence.

Vaccinations currently required are as follows:

 Mumps
 Measles
 Rubella
 Polio
 Tetanus
 Diphtheria
 Pertussis
 Haemophilus influenzae Type B
 Rotavirus
 Hepatitis A
 Hepatitis B
 Meningococcal disease
 Varicella
 Pneumococcal
 Influenza
 Any other vaccinations recommended by the Centers for Disease Control (CDC) and
Advisory Committee for Immunization Practices.

Note: On December 14, 2009, the 2009 Technical Instructions for Panel Physicians for Vaccinations
had replaced the 2007 Vaccination Technical Instructions for Panel Physicians. For more

Bring Your Vaccination Records For The Panel Physician’s Review

You should bring records to show that you have already received vaccination against certain vaccine-
preventable diseases. The panel physician will review the vaccination records and will determine if
you meet the vaccination requirement. Only the panel physician can determine which vaccinations
are medically appropriate. This could mean that you may be required to repeat doses of vaccines that
you have already received. Administering a second dose, however, will not endanger your health.

Vaccination Requirement Waiver

Upon the panel physician’s recommendation, a consular officer may authorize a waiver if it is
determined that a required vaccination is medically inappropriate because of your age, health
condition or medical history. The U.S. Citizenship and Immigration Services (USCIS) may also
authorize a waiver if you can establish that compliance with the vaccination requirements would be
contrary to your religious beliefs or moral convictions.

PAUNAWA SA PAGSUSURING MEDIKAL PARA SA MGA


APPLIKANTE NG IMMIGRANT VISA
Sa ilalim ng batas imigrasyon ng Amerika, LAHAT ng aplikante ng immigrant visa ay
kinakailangang sumailalim sa pagsusuring-medikal. Ang resulta ng pagsusuring ito ay umiiral sa mga
konsul at gagamitin para matiyak kung kayo ay karapat-dapat na tumanggap ng visa.

Ipinangasiwa ng Embahada sa St. Luke’s Medical Center Extension Clinic (SLMCEC) ang
pagpapatupad sa tungkuling ito. Ang SLMCEC ay matatagpuan sa 1177 J. Bocobo St., Ermita,
Manila, telepono bilang 521-0020 at 521-8647. Ito ay bukas Lunes hanggang Biyernes (maliban
kung pista-opisyal), mula 7:30-11:30 ng umaga at mula 1:00-4:30 ng hapon. Pagpunta sa klinika,
dalhin ang inyong pasaporte at isang litrato (na may sukat na 2”x2”). Bago pumunta sa SLMCEC,
hinihikayat na magparehistro online sa website ng SLMCEC: www.slec.ph. Ang pagpaparehistro
online ay ay mapapaiksi ang oras ng proseso at magiging mas mabilis at mas mahusay ang pagbisita
sa SLMCEC.

Importante na makumpleto ang iyong medikal isang linggo bago ang takdang araw ng iyong interbyu.
Ang kakulangan nito ay magiging dahilan para hindi kayo mabigyan ng visa o ang pagpaliban at
pagtakda ng inyong interbyu sa ibang araw. Ang resulta ng medikal ay tuluyang ipapadala ng
SLMCEC sa Embahada.

Paglingkod sa Pang-una

Ang pagkasunod-sunod sa mga magpapasuri ay ayon sa kung sino ang unang dumating. Ang
eksaminasyon ay maaaring abutin ng mahigit isang (1) araw para matapos. Ang mga babaeng
aplikante ay maaaring humiling na suriin sila ng isang babaeng doktor. Ang klinika ay hindi-gaanong
kalakihan ang lugar, kaya huwag magsama ng ibang tao kung hindi kinakailangan.
Bayad sa Medikal

Ang bayad sa pagsusuring-medikal ay US$223.35 para sa mga aplikanteng labing-limang (15) taong
gulang at pataas, at US$185.35 naman sa mga batang labing-apat (14) na taong gulang at pababa.
Ang pagpapa-ulit ng X-ray ay US$7 bawat isa. Maaaring bayaran lamang ang katumbas ng mga
ito sa Philippine pesos sa kasalukuyang halaga ng palitan.

X-Rays
Ang chest X-ray ay hindi kailangan sa mga batang wala pang labing-limang (15) taong gulang,
maliban kung sila ay may sintomas ng tuberkulosis (TB) o posibleng pagkahawa ng TB, halimbawa
dahil sa pakikisalamuha sa kasambahay na may TB. Ipinag-uutos ng U.S. Public Health
Service/Centers for Disease Control (USPHS/CDC) ang pagpagpa-X-ray ng mga buntis. Ang chest
X-ray sa mga buntis ay hindi mapanganib kung ito ay isasagawa ng may tamang panabing. Ang
resulta ng mga nauna o dating chest X-rays ay kinakailangan ding dalhin para masuri at maihambing.
PAGPAPABAKUNA NG MGA APPLIKANTE NG IMMIGRANT VISA

Sa ilalim ng batas-imigrasyon ng Estados Unidos, LAHAT ng mga nagnanais na pirmihang


manirahan doon ay kinakailangang mabakunahan laban sa ilang mga sakit.

Ang mga kinakailangang bakuna ay ang mga sumusunod:

 Mumps
 Measles
 Rubella
 Polio
 Tetanus
 Diphtheria
 Pertussis
 Haemophilus influenzae Type B
 Rotavirus
 Hepatitis A
 Hepatitis B
 Meningococcal disease
 Varicella
 Pneumococcal
 Influenza
 At mga bakunang ini-rekomenda ng Centers for Disease Control (CDC) at Advisory
Committee for Immunization Practices

Dalhin Ang Talaan Ng Inyong Bakuna Para Mapag-aralan Ng Doktor


Kailangang magdala ng katunayang kayo ay dati ng nabakunahan. Pag-aaralan ng doktor ang talaan o
ulat ng inyong bakuna para matiyak na ang mga ito ay sapat ayon sa batas. Tanging ang doktor ang
makakapagsabi kung ano ang nararapat na bakuna. Maaaring ipaulit sa inyo ang pagkuha sa mga
dating bakuna. Tandaan na ang pagkuha ng pangalawang dosis ay hindi makakasama sa inyong
kalusugan.

Waiver sa Bakuna
Batay sa rekomendasyon ng doktor, maaaring pahintulutan ng konsul ang waiver kung
mapapagpasiyahan na ang pagpapabakuna ay hindi angkop dahil sa inyong edad, kalusugan o
nakaraan na may kinalaman sa pagpapagamot. Ang U.S. Citizenship and Immigration Services
(USCIS) ay maaari ding magbigay ng waiver kung mapapatunayan na ang pagpapabakuna ay taliwas
sa inyong paniniwalang pang-relihiyon o matibay na paniniwalang pang-asal/pag-uugali.
