Data Collection Instrument

The researchers used a survey questionnaire composed of four parts to determine grade 12 students' understanding of modular distance learning. Part 1 included 15 rating questions, part 2 had 14 dichotomous terms and conditions, part 3 was a 9 item rating scale, and part 4 collected data on subjects from the 1st semester. The questions addressed statements in the research problem involving students' level of understanding of modular distance learning. The questionnaire was approved by an adviser before being distributed to respondents.

Data Collection Instrument

Since the purpose of the research was to determine the level of understanding on modular

distance learning among grade 12 students of Kapangan Central National High School to the

respondents, the researchers used a survey questionnaire in gathering all the necessary and

desired data. The data gathering instruments for part 1 is a likert scale type of questionnaire

which is composed of rating questions with 15 items, part 2 is the dichotomous questions using

14 terms/conditions, part 3 is the rating scale with 9 subject rate, and part 4 is to provide the

needed data which contains the 1st semester subjects, we know that we should kept our promise

to keep the grades confidential.

The questions were based from the statements of the problem and the body of the

research which was able to address the statement of the problem involving the research entitled

“Level of Understanding on Modular Distance Learning among Grade 12 students of Kapangan

Central National High School. The questionnaire was given to the adviser for checking and

approval before floating it to the respondents.
