Evidence based practice
Introduction -Evidence based practice is a problem solving approach to the delivery of healthcare
that integrate the best evidence from studies and patient care data with clinician expertise and Patient
preferences and values.
Evidence based practice is the integration of clinical expertise present values and the best research
evidence into the decision-making process for the patient care.
Nicole expertise refers to the clinic clinician simulated experience education and clinical skills
Definition-Evidence based practice as using the best evidence available to guide clinical decision
making to shift the provision of healthcare away from opinion past practices and precedent toward a
more scientific basis

evidence from the nursing and medical literature to improve the quality of clinical nursing practices.

The following five steps are
1.Formulating a clear question based on a clinical problem
A. Problem based sources-In this category the healthcare professionals get an idea about relevant
question for evidence based practice from the clinical or risk assessment data such as data of quality
improvement risk surveillance benchmarking data financial data recurrent clinical problems et cetera
for example increased incidence of pressure sore in critical care units may be one of the important issue
to be considered for EBP
B. Knowledge based source-Healthcare professional my often knowledge of newer evidence from the
scientific journals publications research conferences.

2. Literature review two search best available evidence
This is the second step of evidence based practice process bare essential and relevant literature related
to the topic is extensively reviewed and it takes a review for erase based practice must start with
theoretical and clinical article to have overview about the topic of concern that should be followed by
well known existing evidence based practice guidelines.
3 .Analysis of strengths and weaknesses of evidences
Use of rating system to determine the quality of the research is crucial to the development
of evidence based practice there are several rating scale proposed to determine the validity
strength and weakness of the available evidence
4. Use of best evidence in clinical practices
Internal and external entity Once study findings are analysed for strength weaknesses but the season is
made about appropriateness of evidence for the particular question initiated for evidence based practice
the final evidences must be extensively discussed among the users with risk benefit ratio of evidence
and extent and level of harms are involved with particular evidence
5. Evaluating the efficacy of empirical evidences.
Finally after the implementation of the useful findings for the clinical practices efficacy and
performance is evaluated through process of self reflection international audit or peer assessment.
Purposes of evidence based practice
1. Evidence based practice seeks to replace practice as usual with practice guided by rigorous
outcomes oriented research ideally randomised control trials
2. Its goal is to eliminate unsound or excessively risky practices in favour of those that have
better outcomes.
3. Produces the variations in nursing care an assist with efficient end effective decision
making
4. Provide practice do the nurse evidence based data to deliver effective care
BARRIERS IN EBP
 Overwhelming amount of information unable in the literature and sometimes contradictory
findings of the research studies further clear the confusion among practitioners
 Lack of professional ability two critically apprise research this include having a considerable
amount of research evaluation skills access to general and clinic / hospitals support who spend
time on evidence based nursing are limited for the nurses
 Lack of time workload pressure andCompeting priorities of patient care.
 Lack of knowledge of research methods is also one of the significant barrier being promotional
evidence based practice
 Black Ops support from professional colleagues and organisations and lack of confidence and
authority in the research area.
SUMMARY
In evidence based practice , decisions about patient care are guided by three kind of information
 Best research evidences
 Clinical expertise
 Patient values and circumstances
There are five keys in EBP are
1. Ask
2. Acquire
3. Appraise
4. Apply
5. Assess
The purpose is to improve quality, effectiveness and appropriateness of clinical practice.
