AFFIDAVIT

I, SUSANA BEGUERAS DISCAYA, of legal age, widow, Filipino and a


resident of Brgy., Kenram, Isulan, Sultan Kudarat, after having been duly sworn to before
competent authority hereby deposes and states that:

1. That I am the widow of my late husband EDDIE TOGONON DISCAYA;

2. That he died on December 6, 2014 in our residence, Brgy., Kenram, Isulan,


Sultan Kudarat;

3. That he was a member of the Armed Forces of the Philippines(AFP),


Philippine Navy(PN) Marines(M);

4. That I executing this affidavit to terminate my late husband’s account at


Armed Forces and Police Loan Association Inc., (AFPLAI); and

5. That I am executing this affidavit to attest to the truth of the foregoing facts
and for whatever purpose this may serve best.
