AFFIDAVIT OF LOSS

I, CONRADO SANTOS, of legal age, married, Filipino, and a resident


of Candon, Ilocos Sur, after having been sworn in accordance with law, hereby
depose and say that:

1. I am the holder of an ATM card issued by Land Bank of the Philippines,


Candon Branch, under account bearing number 00001234567;

2. I kept the said ATM card in my wallet which I always bring with me
every time;

3. Last 6 April 2021, I needed my ATM card to withdraw cash from my


account, however, to my surprise, I could no longer find it in my wallet;

4. Consequently, I immediately exerted all diligent efforts in looking for the


said ATM card but until now, I have not found the same, for which
reason the said ATM card may now be considered as irretrievably lost;
and

5. I execute this Affidavit to attest to the truth of the foregoing facts.

IN WITNESS WHEREOF, I have hereunto set my hand this 7th day of


April 2021 in Candon Ilocos Sur.
