UNDERTAKING AGAINST JOINING
FRATERNITIES/SORORITIES
Effective Date: July 28, 2017

To the applicant: Submission of this signed statement to the Office of the Registar is a requirement for
enrollment at the Mapúa University.

I, ________________________________, ___________ citizen, _______ years old, with residence at


_______________________________________________________________, do hereby voluntarily and knowingly
undertake to do the following as part of the requirements for my admission to Mapúa University, as well as part of the
conditions for my retention in and graduation from the school:

1. I confirm that I am not a member of any fraternity, sorority, or other organization of similar nature or purpose;

2. Throughout the duration of my stay in Mapúa, I shall not join, form, or organize any fraternity, sorority, or other
organization of similar nature or purpose which is not recognized by the University. Neither shall I participate,
assist, or support their activities in any manner;

3. I acknowledge and understand that the following are considered major offenses under the Student Handbook:
(i) forming and joining any fraternity, sorority, or other organization of similar nature or purpose and not
recognized by the school; (ii) hazing or infliction of any physical or mental harm on any person, or subjecting any
person to any ordeal which may or may not injure, disgrace, or tend to injure, degrade, or disgrace any fellow
student or person. “Harmful acts” include the conduct of initiation as a requirement for admission to any
organization, whether recognized or not by the school;

4. I understand that, if I commit any act which contradicts or violates any of the foregoing, I will undergo disciplinary
proceedings and the school may impose on me the appropriate penalty stated in the Student Handbook which
may include suspension or dismissal from the school, depending on the circumstances of the case;

5. I likewise undertake to read, understand, and comply with all student rules and regulations as contained in the
Student Handbook, the Cardinal Guide, and in other official documents that are hereinafter published in the
school website.
