Resulting to Damage of Property

1. References:
a. Affidavit of Complaint of Pat Rodney Bantolo dated September 28,
2021;
b. Affidavit of Witness of Eric Pagui dated September 28, 2021
c. Certificate of Non-forum Shopping Submitted on September 28, 2021
d. Notice of Complaint sent to respondent Engr. Ulo Malaki dated
September 29; and
e. RA 1790.

2. This pertains to the initial evaluation conducted by this office on the


administrative complaint against Engr. Ulo Lang Malaki of Tagaytay City for reckless
imprudence resulting to damage of property (motorcycle).

3. Records show that Engr. Ulo Lang Malaki recklessly and imprudently
tried to make U-turn and hit a parked motorcycle owned by Pat Rodney Agting
Bantolo.

4. The Investigator on case obtained CCTV footages and Blotter of the


Traffic Division of Silang MPS.

5. In view of the foregoing, request approval for the conduct of Pre-Charge


Investigation against Engr. Ulo Lang Malaki of Tagaytay City to determine whether
there is probable cause to warrant the filing of formal charge.

6. Respectfully recommends approval of paragraph 5.
