Instruction for Photo & Signature

1. Note: Sign with Black ink Pen


2. Paste 2 Photographs, 3.5 cm X 2.5 cm (Refer - Image 1)
3. On the left hand side sign across the photo (Refer - Image 1)
4. On the right hand side sign below the photo within the Box (Refer - Image 1)
5. Sign at the bottom of the page within the Box (Refer - Image 2)

6. Once form is ready, it should be send along with self-attested document proofs for
identity, Address and date of Birth by Courier or Speed Post to the mentioned address
i.e.

PAN Service Centre,
Address: kirti Bhawan, CTS No. 335/2, Shilavihar Colony, S.No. 42/43, 3rd
Floor,Paud Road, Kothrud, Pune, 411038, Maharashtra, India

7. The PAN Application process would start once we received the duly signed form along
with all the Document Proofs.
8. You would be intimidated by an email with your acknowledgement number after
receiving the signed application form along with ID Proof, Date of Birth Proof &
Address Proof.
