Objectives:
Within 8 hours of clinical duty as a student head nurse at Baguio General Hospital and
Medical Center (Medical Ward) will be able to:
- Demonstrate a positive behavior in the outcome of event during their exposure to
client.
- Have a good time management which is well delegated and having equality of
distribution of responsibilities and cases for the rest of the group
- To promote an individualized primary care to attend the needs of the patients.

6:45-7:00 am Arrival at the waiting area
Checking of attendance
Checking of paraphernalia, and uniforms
7:00-7:30 Participate in the endorsement done by health care team members in the
morning shift
7:30-7:45 am Delegation of areas of responsibilities
7:45-8:00 Receiving patient, monitoring vital signs
Checking of IV fluids and administering of meds
8:30-9:30 Checking patient’s chart for doctors order
9:30-11:00 Taking vital signs
Sample and initial checking of FDAR
11:00-11:45 Break for the 1st batch
Monitor meds of patients
11:45-12:25 Break for the 2nd batch
12:25-1:00 Final checking of FDAR together with the C.I and transferring it to the metal
chart
1:00-1:30 Monitoring of vital signs
1:30-2:00 Final recording of vital signs, I & O
2:30-3:00 Post conference

Objectives:
Within 8 hours of clinical duty as a student head nurse at Baguio General Hospital and

- Students provide health education based on identified learning needs of the client.
- Facilitate nursing rounds together with the student nurses
- Checking vital signs, IVF level, I&O results for entire course of the shift patients chart
- Students communicate therapeutically with the client and their significant others

Checking of attendance, uniforms and paraphernalia
7:00-7:45 Joining for the morning prayer
Participate for endorsement
7:45-8:00 Assigning students to their to their patients
8:00-8:30 Checking/monitoring vital signs
Checking and administering of medications together with the students
Checking/ regulating of IV fluids
8:30-9:00 Assessing patient needs
9:00-9:20 Monitoring vital signs
Checking patient’s chart for doctor’s order
9:20-10:30 Sample charting of students
Initial checking of FDAR
10:30-10:45 Short break
10:45-11:15 Vital sign monitoring
Nursing interventions
11:15-12:00 Lunch break (1st batch)
Monitor afternoon meds of patient

Objectives:
At the end of the rotation, students will be able to attain relationship among the other
students, clinical instructor and other health care personnel in which learning is facilitated
towards attainment of competency in rendering quality care to clients within the healthcare
delivery setting and having equality of distribution of responsibilities and cases for the rest
of the group.
