Dear Sir

As per the State Government Guidelines, I came to know that students are permitted to attend
institute for classes in physical mode from 1st September, 2021 with the written consent of parents.
I hereby extend my consent for my ward…………………………………………… (Son/daughter)
studying in Semester…... Branch & Section…………………………… to be allowed for attending
classes in offline mode scheduled from 1st September, 2021 as per the institute guidelines.
In addition, I declare that my ward is in good health and is not undergoing any medical treatment.
I also promise that if my ward is diagnosed with Covid 19, I will contact the institute authorities
immediately.
I am also aware that in case of my ward getting any symptom of Covid-19 or having Covid-19
positive during the days of attending offline classes, the institute will not be held responsible for it.
Also my ward will follow the Covid-19 safety guidelines as stated by Institute or State/Central
Government from time to time.
