Definition
Dressing: is a sterile pad or compress applied to wound
Wound: is a type of injury which happen relatively quickly in which skin is turn
out or punctured
Wound dressing: can simply be defined as the total hygiene given to a wound so
as to prevent contamination and promote healing
Principle involve in wound dressing
1. Explain procedure to the patient to the level of his understanding and gain
patient consent
2. Adequate hand washing before and after dressing
3. Close nearby windows and minimize human traffication
4. Sweeping and dusting should be done an hour before dressing
5. Dressing involve two nurses procedure; dresser and assistant
6. Nurse that is having upper respiratory disease condition should not be
allowed to dress
7. Dressing is non touch procedure
8. The dresser should swipe the wound once by using swab
9. When applying bandage it should not be so tight as this will reduce the
amount of blood flow to the area
10. Provide a privacy
Purpose of wound dressing

5. To promote patient comfort
6. To remove dead tissue
7. To remove foreign body
8. To improve blood supply to the area

Requirement
Trolley is required
Top shelf; all requirements are sterile
Large tray with lid containing the following
Three dressing forceps
One pair of dissecting forceps
Surgical scissors
Three galli pots
Bowl with cover containing dressing towel and mackintosh
Bottom shelf; all requirements are clean
Large tray containing the following
Bottles of lotions
Bandage
Plaster
Counter scissor
Latex examination glove
Face mask
vaseline guaze
six inch cotton bandage
large receiver for used instruments savlon 1: 20
pedal bin
Method of the procedure
A. The dresser explain procedure to the patient and get the patient consent
B. Assess the patient wound
C. The dresser and the assistant scrub their hand and put on mask and gloves
D. Clean the trolley with disinfectant
E. The dresser will set the top shelf requirement using cheatle forceps and
instruct the assistant on setting bottom shelf
F. The dresser and assistant provide privacy, position to the patient and put off
the fan
G. The dresser wheel the trolley to the patient bed side while the assistant carry
the pedal bin
H. The dresser instruct the assistant to remove the top dressing
I. The assistant open the tray and bowl and pours the lotion after showing it to
the dresser
J. The dresser pick the mackintosh with a pair dressing forceps and spread it
under the area to be dressed without contaminating the forceps and then
place the dressing towel in the same manner
K. The inner dressing is removed before the dressing forceps is discarded
L. Use second dressing forceps to dress the wound first from the cleanest area
to the dirties area using each swab once and only in one direction
M. The wound itself is clean in the same manner using as many swab as needed
and discard the used forceps
N. Prescribed lotion powder etc. is applied to the wound, cover the wound with
sterile guaze using the remaining dressing forceps including dissecting
forceps especially in folding the guaze to remove excess lotion in wet
dressing
O. The assistant is instructed to apply the bandage, plaster as the case maybe
P. The dresser remove all the requirement used from patient bed side and the
patient left in comfortable position
Q. Thank the patient for being cooperative
R. Discard the requirement accordingly
