PNP Health Report Explanation

This memorandum from three Philippine National Police (PNP) officers explains why they could not comply with daily health monitoring procedures on January 4, 2022. Officer Rodrin was instructed to prepare for duty at a quarantine facility after being released from hospital duties. Officer Velonza was also told to get ready for a new quarantine facility assignment. Officer Garcia was busy with duties at the PNP swabbing facility due to increased Covid patients. Officer Andador was on duty at the PNP hospital pharmacy for the same reason. All officers state they did not intend to disregard health protocols and seek consideration from their office.

Subject: Order to Submit an Explanation re: Health Monitoring Report.

2. In connection with above reference, please be informed that the undersigned


could not comply with the PNP CODA that day January 04, 2022 because of the
advance information for her termination of duties from OB ward at PNPGH and
augmentation at Kiangan Quarantine Facility were the undersigned was on going New
Years Break that time and were instructed to prepare things for orientation and duty at
Kiangan Quarantine facility immediately because of the upsurge number of Covid 19
Positive patient.

3. Moreover please be informed that the undersigned has no intention not to


comply with the PNP CODA for Daily Health Assessment and therefore seek before
your good Office for favorable consideration hereof.

information for his termination of duties from Ortho ward at PNPGH and augmentation
at Kiangan Quarantine Facility the undersigned was ongoing duty on Ortho ward that
time and were instructed to prepare things for orientation and duty at Kiangan
Quarantine Facility immediately because of the upsurge number of Covid 19 Positive
patient.

3. Moreover please be informed that the undersigned has no intention not to


comply with the PNP CODA for Daily Health Assessment and therefore seek before
your good Office for consideration hereof.
