AFFIDAVIT OF LOSS
I, ROCHELLE ODSINADA MARQUESES, of legal age, single, Filipino, and a
resident of Unit 360 Tanglaw Bldg, 150 Tanglaw, Plainview Mandaluyong City,
Philippines, after having been sworn to in accordance with law hereby depose and state
that:
1. I am licensed driver holding a Driver’s License with License No. EO2-13-
000862 issued by the Land Transportation Authority (LTO) – Driver’s License
Renewal Center in Mandaluyong City.
2. On 01 February 2022, I was on duty and responded to a fire call. Then, upon
arrival to the location, I left my small bag with my wallet in the fire truck. After
the fire has been declared fire out I proceeded to the fire truck to get my thing
and thereafter realized that my wallet containing all my identification card
including my driver’s license was missing
3. I took diligent search and efforts to locate my wallet but I could not find it,
hence I believe that it is now beyond recovery.
4. I am executing this Affidavit of Loss to attest to the truth of the foregoing
statement, and to request for a replacement of my Driver’s License and for
whatever legal purpose it may serve.

DEED OF ABSOLUTE SALE
Known All Men By These Presents:
This DEED OF ABSOLUTE SALE made and executed on this 3rd day of
February 2021 in Mandaluyong City, by and between:
JUAN DELA CRUZ, Filipino, of legal age, single with address No.60 Antonio St,
Addition Hills, Mandaluyong City, herein referred to as “VENDOR”
-and-
MARIA DELOS REYES, Filipino of legal age, single and with address at De
Jarme St., Brgy. Bayanihan Dolores Quezon Province, herein referred to as the
“VENDOR”.
The VENDOR and the VENDEE are collectively referred to as “Parties”
Witnnesseth, that:
I. WHEREAS the VENDOR is the owner of the following described real property
covered by Transfer Certificate of Title No. xxxxxxxxx of the Registry of Mandaluyong
and more particularly described as follows:
A PARCEL OF LAND XXXXXX, XXXXXXXX XXXXXX
CONTAINING AN AREA OF THREE HUNDRES (300)
SQUARE METERS, MORE OR LESS XXXXXXXXXXX
II. WHEREAS the VENDEE aggress to purchase the said property subject to the
following conditions:
NOW, THEREFORE, for and in consideration set herein, the Parties agrees as
follows:
1. Re Purchase Price:
A. Total of purchase price for the realty subject of this deed is TWENTY EIGHT
MILLION PESOS (Php 28,000,000.00) payable upon the execution of this
deed via cash;
2. Re Delivery of Documents: The VENDOR, shall deliver and signas may
reasonably necessary, document needed to take effect this Deed, specifically for
the purpose of effecting in the issuance of a title in the name of the VENDEE, his
heirs, successors or assigns;
3. Re Payment of Capital Gains, Etc.: The VENDOR will shoulder the standard % of
Capital Gains Tax while all other taxes and fees will be shoulder by the VENDEE.
IN WITNESS WHEREOF, I have hereunto signed this deed of sale, this 3rd day of
