I, LYDIA HINAUT MAGHANOY-ANDAYA, Filipino, of legal
age, widow and resident of Dao, Pagadian City, Zamboanga del
Sur, after having duly sworn to in accordance with law, do
hereby depose and say;
That I am a law abiding citizen; and;
That I have a son whom we named ROLLY M. ANDAYA. He was
born on May 11, 1992. As such as Certificate of Live Birth
was issued as proof of the fact of his birth;
That however, I the said Certificate of Live Birth, my
maiden middle name was erroneously spelled as “Hinaot”
instead of “Hinaut”;
That Lydia “Hinaot” Andaya as appearing in the
Ceritificate of Live Birth of Rolly Maghanoy Andaya and
Lydia “Hinaot” Andaya are one and the same person;
That this erroneous entries, if not corrected in time
will cause confusion as to my personal and future
transaction;
That I am executing this affidavit to attest to the
truth of the foregoing statements;

depose and say;
That I am a law abiding citizen; and;
That I was baptized at San Jose Parish, Midsalip,
Zamboanga del Sur but unfortunately the entry of my first
name as appeared in the record from said Parish does not
coincide with the records of my Certificate of Live Birth
duly issued by the Local Civil Registrar and the NSO SECPA,
said erroneous entry as appeared in the PARISH was PERLIE H.
LOMOLOY instead of PERLY H. LOMOLOY per Certificate of Live
Birth and used in all of my transactions;
That this erroneous entries, if not corrected in time
will cause confusion as to my personal and future
transaction;
That I am executing this affidavit to attest to the
