requirements for the said activity are complied with.

However, when untoward incidents occur beyond the control of the school and the Work Immersion
Partner Institution, despite all the efforts taken and the exercise of due diligence effectively
established, I waive and release the Bernardo D. Carpio National High School, its administrators,
faculty, and personnel, and the Work Immersion Partner Institution from liability. Accordingly, I shall
fully cooperate with Bernardo D. Carpio National High School and the Work Immersion Partner
Institution for the reason of any untoward and uncontrollable events.
