TWO DISINTERESTED PERSONS
(Application: Late Registration of Birth Certificate)

We, ______________ and _______________ both of legal age, Filipinos, both


married and residents of Subida, Singsing and Proper Cantuod, Balamban, Cebu, after
having been duly sworn in accordance with law, do hereby depose and say, T H A T:

1. We personally know parents ______________and ____________________, and


their child ______________ (Andrea, for brevity) because we are neighbors for a
long time.

2. Andrea Antipuesto was born on June 22, 2020, at Sitio Tunga, Barangay Cantuod,
Balamban, Cebu. Her birth certificate was not registered within the reglementary
period for reason that at the time of birth it was during the series of lockdowns
implemented in Balamban, Cebu.

3. We are not in any way related to ____________ by affinity nor consanguinity;

4. That finally, we are executing this affidavit for the purpose of informing the
authorities especially the Local Civil Registrar and the Philippine Statistics Authority
of the above-mentioned and to support the application for late registration of birth
certificate or DELAYED REGISTRATION of ______________ who is the child of
___________________ This affidavit is based on personal knowledge.

IN WITNESS WHEREOF, we hereby affix our signatures, this 8 th day of July 2022 in
Municipality of Balamban, Cebu, Philippines.
