APPEARANCE
Mari Jane Cruz, 24 years old is an average girl. She’s very simple and
she’s not wearing any make-up. She’s wearing a black shirt and
eyeglasses.

II. BEHAVIORAL OBSERVATION


(Motor Activity, Speech, Mood, and Affect)

At the beginning of the interview, she looked very relaxed and she was
open to all the questions. Mari Jane’s mood was observed to be normal.
She was cooperative and attentive throughout the evaluation and gives her
best effort to answer all the questions. No unusual mannerisms, abnormal
movements, tics, or tremors are noted. and her eye contact was within
normal limits. Her tone of voice is appropriate and soft. Also, Mari Jane
seems to be euthymic throughout the interview.

III. CLINICAL INTERVIEW

Thought Content Mari Jane talks about her family and being the breadwinner, which
possibly causes her stress. She mentioned being an anxious
person and she always worries about her future, during this time
she experienced physical symptoms like increased heart rate,
trembling, and panic attacks. Also, she indicated that her study
distracts her from her overthinking. She denies any form of suicide
or self-harm.

Thought Process Mari Jane answered the question in a goal-directed manner. Her
thoughts were organized and coherent.

Cognition Cognitive functions are intact for orientation and consciousness to


place, person, and day. Mari Jane is attentive and focused during
the interview and she answered immediately, rationally, and
intelligently.

Insight and Mari Jane mentioned a therapist who gave her a piece of advice.
Judgment She said “Kung ano yung nararamandaman mo, hayaan mo lang
yung sarili mo na maramdaman yan, tanggapin mo kung ano yung
nararamdaman mo.” What she felt about being a breadwinner and
her responsibility to her family and her worries about her future
were also noted. But she is coping through mindfulness
techniques, like self-meditation. Mari Jane showed a strong drive
to finish her study in order for her to achieve all her goals and
ambition in life.
