All claim forms must be duly completed and submitted together with required documents to avoid delay in claims
processing. / Semua borang tuntutan perlu dilengkapkan dan dihantar bersama dengan dokumen-dokumen untuk
mengelakkan kelewatan dalam pemprosesan.

All documents must be checked by branch PIC/staff prior to submission. / Semua dokumen perlu disemak oleh
kakitangan cawangan/pihak yang bertangungjawab sebelum penyerahan.

Subsequent bill/receipt must be submitted with a fresh Claim Form and Claim Checklist. / Bil/resit seterusnya
mesti dihantar bersama Borang Tuntutan dan Senarai Penyerahan Tuntutan yang baru.

Death Claim / Tuntutan Kematian


1. Death due to suicide within first year from the Certificate/Annexure or the Revival Date is not payable./
Manfaat kematian tidak akan dibayar sekiranya kematian Orang yang Dilindungi adalah akibat bunuh diri yang mana
manfaat terpakai dalam tempoh 1 tahun dari tarikh permulaan manfaat kematian atau mana-mana tarikh Sijil/Lampiran
diaktifkan semula.
2. All certification of documents as true copy must be by an Executive of PruBSN. / Semua pengesahan dokumen
mesti dibuat oleh Eksekutif PruBSN.

TPD Claim / Tuntutan Hilang Upaya Penuh Dan Kekal


Disability arising out of the following is not covered. / Hilang Upaya Dan Kekal tidak akan dibayar sekiranya disebabkan
oleh :
1. Attempted suicide or self-inflicted while sane or insane. / Percubaan membunuh diri atau kecederaan yang
dilakukan sendiri semasa siuman atau tidak siuman.
2. Engaging in aerial flights including parachuting and sky diving, other than as a crew member of or as a fare
paying passenger on a licensed carrying commercial aircraft operating on a regular schedule route. / Melakukan
penerbangan udara termasuk penerjunan berpayung dan penerjunan udara selain daripada sebagai anak kapal atau
penumpang yang membayar tambang di atas kapal terbang pembawa penumpang komersial yang berlesen dan beroperasi
dalam latihan yang berjadual.

Crisis Cover Claim / Tuntutan Perlindungan Krisis


The symptoms of the illness must manifest after 60 days from the Certificate/Annexure Date or Revival Date of
Endorsement or Commencement Date of the benefit, whichever is the latest. / Tanda-tanda penyakit kritikal mesti
timbul selepas 60 hari dari Tarikh Sijil/Pengeluaran Lampiran atau Tarikh Berkuatkuasa Semula atau Tarikh Pengubahsuaian
atau Tarikh Permulaan manfaat ini, yang mana terkini.

WI Claim / Tuntutan Imbuhan Mingguan


Weekly Indemnity is not paid according to Medical certificate issued to the Person Covered. / Imbuhan Mingguan
bukan dibayar mengikut Sijil Cuti Sakit yang diberikan kepada Orang yang Dilindungi.

AMR Claim / Tuntutan Pembayaran Balik Kos Perubatan Kemalangan


All receipts submitted would not be returned to the Certificate Owner if they are duly reimbursed by PruBSN. /
Semua resit yang diserahkan tidak akan dikembalikan kepada Pemilik Sijil sekiranya telah dibayar balik kos perbelanjaannya
oleh PruBSN.
