OF CHANGE OF COLOR OF MOTOR VEHICLE

I, DONNA ANTOINETTE A. CASIO, of legal age, Filipino, married, and a resident of


New Road, Brgy. Mongpong, Roxas City, Capiz and ADRIAN RAPHAEL A. ESPINOSA, of
legal age, Filipino, and a resident of 63 E. Lopez St. Jaro, Iloilo City, after having been duly
sworn to in accordance with law, hereby depose and state that:

1. I, DONNA ANTOINETTE A CASIO, am the registered owner of a certain motor


vehicle particularly described as follows:

Make: HONDA Engine No.: P6DD1-P402069


Body Type: SEDAN Chassis No.: PADEK3520XV102031
Color: GOLD-SILVER Plate No.: DTR495

2. The original color of the said motor vehicle was “Gold-Silver”;

3. When the paint of the above-described motor vehicle fade, I had its color repainted
to “Battleship Gray” such that the color of the motor vehicle was changed from
“Gold-Silver” to “Battleship Gray”;

4. The said paint job was done by ADRIAN RAPHAEL A. ESPINOSA;

5. The change of color is not intended for any illegal or unlawful purpose but solely due
to the foregoing reasons;

6. I am executing this Affidavit to attest to the truth of the foregoing and in support of
the application for the change of color of the above-described motor vehicle on its
Certificate of Registration and for whatever legal purpose this may serve.
