CERTIFICATION
This is to certify that IAN P. TAPANAN was truly present for the whole day in
March 03, 2022. This also certifies that the double entry for time of arrival (Time-In
Morning) appeared in the biometric printout.
Unfortunately, to the best of his/her knowledge, he/she logged in to the biometric but
his/her record on the afternoon(Time-out) were not reflected when the print-out of the
machine came. Attached herewith is the photocopy of the logbook to further justify his/her
attendance.

Given this 4th day of April, 2022 at San Vicente Integrated School, San Vicente,
Montevista, Davao de Oro.
