AFFIDAVIT OF LOSS
I, CLAYTON SPREWEL T. DIOLA, of legal age, Filipino, single, and presently residing
at Florence Subdivision, Brgy Cubacub, Mandaue City, after having been duly sworn to in
accordance with law, do hereby depose and state, THAT:

1. Last July 9, 2022, upon arrival at Florence subdivision, Brgy. Cubacub, Mandaue,
Cebu from SM City Cebu, I discovered that my Driver’s License was already
missing.

2. I am of the firm belief that my said driver’s license fell from my said built bag
without my knowledge and could no longer find them;

3. Since then, I have exerted diligent and honest to goodness efforts in order to locate
the said documents but all proved futile as I apparently could no longer locate the
same;

4. Considering the foregoing and for all intents and purposes, the said documents are
now lost and already beyond recovery;

5. I am executing this Affidavit in good faith to attest to the fact of loss of said
documents and to attest to the truth and veracity of the foregoing averments and
for whatever legal intent and purpose the same may serve.

IN WITNESS WHEREOF, I have hereunto affixed my signature this 11 th day of July 2022,
at Mandaue City, Cebu, Philippines.
