IN RE: PETITION FOR COMMISION AS NOTARY PUBLIC

Good afternoon your honor, I’m Atty. _______________________ respectfully


appearing as the petitioner in this case and I am ready to present and
mark my documentary exhibits to establish the jurisdictional facts:

In compliance with the 2004 Rules on Notarial Practice, and to prove


that I am a duly registered member of the Integrated Bar of the
Philippines in good standing and that I have never been convicted in any
court of any crime involving moral turpitude your honor, may I move
that my:

Certificate of Membership in the Philippine Bar issued by the Office of


the Bar Confidant (Annex A) be marked as Exhibit A;

Certificate of Good Standing from the Office of the Bar Confidant,


Clearances from Municipal Trial Court in Cities, Laoag City, Regional
Trial Court and National Bureau of Investigation (Annexes B, C and D
respectively) be marked as Exhibit B, C, and D respectively;
