The patient reported back pain and had a temperature of 36.1°C, pulse of 71, respiratory rate of 20, and blood pressure of 100/70. They were diagnosed with a urinary tract infection. Urinary tract infections are caused by bacteria entering the urethra and bladder, and can spread to the kidneys. The nursing care plan was to increase fluid intake, encourage voiding at regular intervals, and administer prescribed medications to achieve normal urination patterns within 24 hours by decreasing urinary frequency, urgency, and fatigue.

Diagnosis Impaired urinary elimination related to urinary tract infection

Inference Urinary tract infections are caused by pathogens, usually bacteria that enter the urethra and then the bladder. This can lead to infection, most commonly in the bladder itself, which can spread to the kidneys.

Planning After a series of nursing intervention, ( fluid intake, voiding regular intervals administer needs as prescribe. The client will be able to achieve normal elimination patterns as manifested by frequency, urgency of urination, lighter color urine and decrease restlessness and fatigue within 24 hours.

Interventions Independent: 1.Encourage fluid intake up to 3000ml/day
