AFFIDAVIT

I, YYY, of legal age, married, Filipino, and a resident of 11-B Perimeter St., Pag-
asa, Olongapo City, after having been sworn in accordance with the law hereby depose
and state that:

1. I am currently employed as an Engineering Assistant at the SBMA Land


and Asset Management and Development Division (“LAMDD”).

2. On May 3, 2021, the Olongapo Electricity Distribution Company, Inc.


(“OEDC”) publicly announced in its Facebook page the scheduled power supply
interruption in Olongapo City on May 10, 2021 (“May 10 power supply interruption”).
Since the May 10 power supply interruption fell on a Monday, the first day of a busy work
week, my wife and I decided to prepare and iron all office clothes and uniforms on
Sunday, May 9, 2021.

3. On May 9, 2021, at around 9:30 p.m., while I was gathering the office
clothes and uniforms at the clothes line, I inadvertently stepped on the tail of a stray cat.
As a result, the stray cat bit my leg. We immediately called my brother-in-law, Mr. Cecil
R. Pineda, Jr., to drive me to the nearest hospital for the administration of an anti-rabies
vaccination shot. My brother-in-law drove me to Mother and Child General Hospital at
Kessing St., Olongapo City (the “Hospital”).

4. The Hospital staff immediately administered one (1) shot of anti-rabies


vaccination into my arm. I was informed that I need a total of five (5) shots of anti-rabies
vaccination within a period of 28 days in order to complete the treatment.

5. I am executing this affidavit to attest to the truth of the foregoing facts, and
for all legal intents and purposes it may serve.

IN WITNESS WHEREOF, I have hereunto set my hand this 12 th day of May,


2021 in Olongapo City, Philippines.
