AFFIDAVIT OF LOSS
KNOW ALL MEN BY THESE PRESENTS:
I, _________, Filipino, single, of legal age and resident of _____________, after having
been duly sworn in accordance with law, do hereby depose and say that:

1. That I am the sole proprietor of _____________ hereinafter referred to as


____________;
2. That the said business employed a bookkeeper who was in charge of keeping and
storing all the books and records of the business of __________ including but not limited
to the following records:

i. List of ending inventory goods and supplies, including capital goods;


ii. Inventory of unused sales invoices and/or official receipts (SI/OR)
iii. Unused sales invoices and/or official receipts and all other unutilized
accounting forms including vouchers, debit and/or credit memos, delivery
receipts, purchase orders, business notices and permits

3. That the said bookkeeper left us for a company outside Metro Manila.
4. When we hired a new bookkeeper, we discovered that the business records were
incomplete.
5. That I tried to search for the said documents, however, I could no longer locate them
despite exertion of utmost diligent efforts;
6. That I am executing this affidavit to attest to the truthfulness of the foregoing
facts and for the purpose of requesting the Bureau of Internal Revenue to
officially close the above-mentioned business and for whatever legal purpose
this may serve.
