MEMORANDUM OF AGREEMENT
Student Internship Program in the Philippines
This Memorandum of Agreement is made and entered into by and between:
The DVD AK 4F GAME FARM, a private farm recognized and existing under the laws of the
Republic of the Philippines, with principal office address at Davao City, represented herein by its
farm owner and veterinarian, DR. HERMIE REY GARCIA, herein after referred to as the,
“COOPERATOR”;
and
The UNIVERSITY OF SOUTHERN MINDANAO (USM), a level IV state University,
created and operating under the laws of the Republic of the Philippines, with principal office
address in Kabacan, North Cotabato, herein represented by its President,
DR. FRANCISCO GIL N. GARCIA, hereinafter referred to as the “USM”.
WITNESSETH
WHEREAS, the USM requires it students to undergo training which is entitled,
Veterinary Clinics 2 (Field Experience) equivalent to “On-the-Job training (OJT)” or
“Apprenticeship”;
WHEREAS, the objectives of the said training are as follows:
1. To develop the student’ ability to integrate knowledge from various disciplines when
identifying and analyzing problems in Doctor of Veterinary Medicine;
2. To expose students to other dynamics in the field of Animal Industry;
3. To sharpen the students’ perception on the use of appropriate Veterinary Tools and
Equipment; and
4. To enable the students to integrate systematically with common sense the knowledge
in his field of endeavor.
WHEREAS, the COOPERATOR, is a private farm engaged in servicing and other
related services and has the facilities to provide trainees with actual exposure in their field;
WHEREAS, the trainees’ residency on the said private farm shall be limited only on
March 9 to March 22, 2020 to provide the trainees a sufficient time to comply and prepare for
the next field experience.
NOW THEREFORE, for and in consideration of the above premises and for the mutual
interest and benefits of the USM, the students and the COOPERATOR, the parties here to agree:

1. The obligations and responsibilities of USM shall be as follows:
a. To implement the training program in the most efficient manner in accordance with the
terms and conditions set forth by both Parties;
b. To designate OJT Coordinator who shall work closely with the cooperator and be
responsible for the training of the students;
c. To supervise the conduct of the OJT in line with the students’ field and see to it that the
OJT is observed properly; and
d. To support financially the travel of OJT Coordinator when visiting/monitoring the
trainees.
2. The duties and responsibilities of the designated OJT Coordination shall be as follows:
a. To be responsible for the screening of the students who shall undergo OJT;
b. To monitor and evaluate the implementation of the work plan;
c. To ensure the safety and welfare of the student trainees
d. To observe strictly the provisions of R.A. 7877, otherwise known as the Anti-sexual
Harassment Law.
3. The obligation and responsibilities of the COOPERATOR shall be as follows:
a. To support, supervise, and assist the student in the implementation of the OJT work;
b. To submit the individual written performance reports and other activities of the students to
the OJT Coordinator;
c. To be responsible for the welfare and safety of the student-trainees during the training
period;
d. To assist the OJT Coordinator when visiting/ monitoring the trainees;
e. To assist the students locate housing, preferably near the area of assignments; and
f. To observe strictly the provisions of R.A. 7877, otherwise known as the Anti sexual
Harassment Law.
4. The obligation and responsibilities of the student-trainees shall be as follows:
a. To support financially the training that includes the travel expenses, board and lodging,
and allowance);
b. To maintain good human relations with the constituents of the cooperating agency and
co-trainees;
c. To obey and follow the house rules of the cooperating agency, and
d. To submit promptly their performance reports and other activities as deemed necessary.
5. It is understood that the OJT is an integral part of the academic work of the students pursuing
the courses leading to the degree of Doctor of Veterinary Medicine at the USM, and the
COOPERATOR is, in no way whatsoever, obliged and responsible for payment of wages and
other emoluments for work done pursuant to the practicum, although voluntary allowances
that maybe provided to the student-trainee are welcome.
6. That this agreement shall commence on March 9, 2020 and shall end on March 22, 2020;
7. That this agreement shall retract from the commencement date mentioned in the preceding
paragraph.
8. That this Memorandum of Agreement shall take effect on the date Above stated and shall
remain in full force until unless otherwise terminated for the following causes:

a. Non-compliance by any of the parties with the terms and conditions set forth in this
Memorandum of Agree, and
b. Upon requested by the USM or COOPERATOR, for the causes beyond their control.
