PERFORMANCE CHECKLIST

1. Washes hands.
2. Assembles Equipment.
3. Introduces self and verifies client’s identity.
4. Explain the procedures.
5. Provide client’s privacy.
6. Position the client comfortably.
7. Inquires any history that affects the eyes.
A. VISION TEST
8. Distant visual acuity (with Snellen chart, normal
acuity is 20/20 with or without corrective lenses).
B. EXTRAOCULAR MUSCLE FUNCTION TESTS
9. Corneal light reflex (using a penlight to observe
parallel alignment of light reflection corneas).
10. Cover test (using an opaque card to cover an eye to
observe for eye movement).
C. EXTERNAL EYE STRUCTURES
11. Inspects eyelids and lashes (width and position of
palpebral fissures, ability to close eyelids, direction
of eyelids in comparison with eyeballs, color,
swelling, lesions and discharge).
12. Inspects positioning of eyeballs (alignment in
sockets, protruding or sunken).
13. Inspect bulbar conjunctiva and sclera (clarity, color
and texture).
14. Inspects palpebral conjunctiva (eversion of upper
eyelid is usually performed only with complaints of
eye pain or sensation or something in the eye).
15. Inspects the lacrimal apparatus over the lacrimal
glands (lateral aspect of upper eyelid) and the
puncta (medial aspect of lower eyelid). Observe
for swelling, redness, or drainage.
16. Palpate the lacrimal apparatus, noting drainage
from the puncta when palpating the nasolacrimal
duct.
17. Inspects the cornea and lens by shining a light to
determine transparency.
18. Inspects the iris and pupil for shape and color.
19. Tests pupillary reaction to light (in a darkened
room, have client focus on a distant object, shine
a light obliquely into the pupil, and observe the
pupil’s reaction to light -normally, pupil constrict).
20. Test accommodation of pupils by shifting gaze
from far to near (normally, pupil constrict).
21. Document findings in the client record.
