Internship Evaluation Guide

This internship evaluation form is to be completed by the student's company supervisor and returned to their university professor. It requests information about the student's name, evaluator/supervisor, their title, and company. It then has a pass/fail grading section for the university advisor. The bulk of the form evaluates the student on personal skills like reliability, ethical behavior, and dedication. It also evaluates their communication, technical, and business skills. Supervisors are asked to rate these on excellence, good, average, or poor. It then asks for comments on the student's internship goals, functional area, projects completed, overall performance, and potential for re-employment.
