SUBJECT: LEGAL NOTICE REGARDING REPUDIATION OF CLAIM OF POLICY
NO. M0000023/C0005723 ON THE LIFE OF LATE MR. MUKESH KUMAR
Dear Receivers,
Under instructions and authority from my Client Mrs. Sunita Devi W/o of Late Mr.
Mukesh Kumar Jalandra, Ward No.11, Choona Chowk, Khetri, Jhunjhunu-333503 And
Her son namely Mr. Mevendra Jalandra who presently residing in Jaipur (hereinafter
referred to as ‘My Client’). I hereby serve upon you, the Noticee, the following legal
notice:
1. By this demand notice, I invite your attention that My Client who submitted a claim
against the policy number M0000023/C0005723. Subsequently, You have repudiated
the claim on the basis that Policyholder concealed material facts pertaining to his
existing medical condition.
2. That it is pertinent to mention here that deceased Mukesh Kumar applied for a loan
from AU Small Finance bank Ltd and for securing the loan, the deceased signed
various paper in presence of your representative in the office of the bank. Your

representative kept blank the application form and just took signatures of the
deceased. You can verify the handwriting of the application form from the
handwriting of your representative. The deceased provided all the information and
documents as per demand of your representative.
3. That you have repudiated claim of my client on basis of private investigation which
not disclosing the cause of death. Your team had done the investigation in haste and
in unprofessionalism manner which gravely adversely affected my client’s mental
and financial condition.
4. That hereby forwarding you all the requisite medical documents which suggest that
Mr. Mukesh Kumar died due to heart attack which is not in your findings.
5. We are constrained to state that aforesaid act from your side prove your intention to
commit and perpetuate fraud on my client and indulge in cheating and criminal
misappropriation.
6. That, the above said act on your part has caused us great loss and damage besides
mental tension, trauma and inconvenience and loss of value of money. This legal
notice may be treated as the last and final communication on the issue as My Client
had already suffered a lot due to your deficiency.
7. Therefore, I call upon you the Noticee to immediately and with utmost expediency
undertake to carry out the following action, namely:
(i) To review the claim of my client against POLICY NO. M0000023/C0005723
ON THE LIFE OF LATE MR. MUKESH KUMAR
(ii) To pay compensation towards My Client in sum of Rs. 1,00,000/- on
account of the mental pain and agony suffered by My Client due to your
default which is totally illegal, arbitrary and unfair trade practice;

(iii) To pay a sum of Rs. 21,000/- towards the cost of the present legal notice.
Since your illegal behavior has necessitated the issue of this notice, you
are hereby charged with its costs.
8. That under these circumstances, You are required to comply with the above
demands forthwith and in any case no later than 15 days from the receipt of this
notice, failing which My Client shall be constrained to initiate appropriate criminal
and civil actions against you as per law, entirely at your risk and cost.
9. A copy of the present legal notice is being retained in my office for the record.
