I , Subir Kumar Das hold the Savings Bank Account No. (21177380765) along with my
wife Mrs Chaina Das in Kolkata Dhakuria Branch. I would further like to state that I also
opened a Term Deposit Account (50391559492) on 06/06/2017 depositing an amount
of Rs.6,20,000 ( Six Lakhs Twenty thousand) for a tenure of the 5 years in your branch.

However, as the term deposit has been matured on 06.06.2022, we urgently need the
money on medical emergency since me and my wife both are very ill and bed ridden.
Being a senior Citizen it is not possible for me to go the bank right now so I am sending
my Nominee who is also my Nephew and family member Mr Rajarshi Das to do the
same.

You are requested to credit my aforesaid Fixed Deposit maturity amount (Principal +
Accumulated interest) to my Savings Bank Account ( 21177380765)

Hoping you would comply to my request at the earliest.
