Greetings of Peace & Solidarity!
I have pleasure of working in IBUAN TRIBAL NATIONAL HIGH SCHOOL, Sitio
Ibuan Brgy. Mampi, Lanuza, Surigao del Sur. I have developed good relations with the
fellow teachers and students there for 3 years and 2 months. However, I cannot go
home daily since the distance is 25 km. away from Carmen to Ibuan so I need to stay
there for a week. It is hard for me to think that I am distant away from my family,
especially that I have a 2-year-old kid who needs my attention.
Thus, I am making this letter of request to transfer to GACUB ATAD S. ILIGAN
TRIBAL NATIONAL HIGH SCHOOL, Gacub Carmen, Surigao del Sur because it is
very accessible to my place of residence, since I am living in Carmen, Surigao del Sur. I
can go home every day and there is nothing to worry for me thinking any anxiousness
towards my family.
Madame, to make it not difficult for you, I have found Teacher I – RUTH F.
TANDOG from GACUB ATAD S. ILIGAN TRIBAL NATIONAL HIGH SCHOOL Gacub
Carmen, Surigao del Sur who is willing to trade station with me. We have the same
major (TLE) and a residence of Brgy. Bunga Lanuza, Surigao del Sur. I think it is
beneficial to the both of us because we will be assigned to our locality. The undersigned
coordinated with her and signed a Memorandum of Agreement (Please see attach
Memo) of our intent to exchange and swap from our post of assignment.
I would be very happy together with my family if you grant me this request. I can
assure you that I can render better service and efficient teaching to my clientele
especially to the department we’re bound to teach lives, nurture minds, and transform

MEMORANDUM OF AGREEMENT FOR THE EXCHANGE OF
WORK AND PLACE ASSIGNMENT
KNOW ALL MEN BY THESE PRESENTS:
WE, SHALYN A. BITUIN, of legal age, married with residence
and post office address Brgy. Poblacion, Carmen, Surigao del Sur
here in after known as the PARTY OF THE FIRST PART, and
RUTH F. TANDOG, of legal age, single, with residence and
post office address Brgy. Bunga Lanuza, Surigao del Sur here in
after known as the PARTY OF THE SECOND PART.
-WITNESSETH-
THAT, the PARTY OF THE FIRST PART is currently a member
of the Teaching Force of the Ibuan Tribal National High School
(ITNHS) as a TLE Teacher, employed by the Department of
Education (DepEd), Caraga Region XIII, Division of Surigao del Sur.
THAT, the PARTY OF THE FIRST PART is requesting the
Division Superintendent of the DepEd, Caraga Region XIII to be
relieve of his teaching workload and be re assigned to Gacub Atad
S. Iligan Tribal National High School at Gacub Carmen, Surigao del
Sur. And,
THAT, the PARTY OF THE FIRST PART main reason of her
request is the accessibility of her place of residence and give full
attention to her family.
THAT, the PARTY OF THE SECOND PART is currently a
member of the Teaching Force of the Gacub Atad S. Iligan Tribal
National High School (GASIT NHS) as a TLE Teacher, employed by
the Department of Education (DepEd), Caraga Region XIII, Division
of Surigao del Sur.
THAT, the PARTY OF THE SECOND PART is requesting the
Division Superintendent of the DepEd, Caraga Region XIII to be

relieve of his teaching workload and be re assigned to Ibuan Tribal
National High School at Sitio Ibuan Brgy. Mampi, Lanuza, Surigao
del Sur. And,
THAT, the PARTY OF THE SECOND PART main reason of her
request is to grant the First Party favor since they have the same
major and the willingness to be assigned in the same place where
she is residing.
THAT, both parties are desirous of exchanging their respective
place of assignment to be able to meet both ends, under which
upon approval of the Division Superintendent of the DepEd, Caraga
Region XIII, Division of Surigao del Sur.
