Patient Categorization

LEVEL 1 LEVEL 2
SELF CARE OF MINIMAL CARE MODERATE OR INTERMEDIATE CARE
PATIENT CAN BATHE, FEED & 1. The patient needs some assistance in
PERFORM ADL'S. bathing, feeding, or ambulating
1. The patient is about to be discharged 2. Up and down for short periods of time
2. One who is non-emergency 3. Extreme symptoms have subsided or
3. One who is newly admitted not yet appeared
4. Does not exhibit unusual symptoms 4. With slight emotional needs

1. The patient is provided a complete bath and
feeding continuous treatments observation,

2. May or may not be unconscious, with marked many medications, IV piggybacks, and
emotional needs vital signs every 15-30mins
3. Vital signs are more than 3x/shift 2. Hourly output, significant changes in
4. With continuous oxygen therapy doctors order more than care hours
5. Needs close supervision for at least 30 minutes
