The undersigned, Grade 12 – STEM Zirconium students of Kitaotao National
High School, are currently processing and pursuing their research study entitled
“COVID-19 Pandemic and Its Impact to the Well-being of Working Students.”
In partial fulfillment of their requirement for the course, Inquiries, Investigations,
and Immersion, the researchers would like to ask permission from your good office to
conduct the study on Kitaotao National High School working students. The study will
involve collecting data from the selected working students with a maximum number of 57
respondents. Thus, the researchers also request for your consent to gather data through
the distribution of survey questionnaires. The researchers will ensure that they will follow
adequate supervision and ethics. Each piece of information that will be collected from
the potential respondents will be strictly confidential and will be solely used for the
needed data in the research study.
