I, LEOPOLDO S. ARCUYO, of legal age, Filipino, married with postal address
at Silway San Juan, Brgy. West, General Santos City, after having been duly sworn to
in accordance with law, do hereby depose and say:
1. That I am a holder of Professional Driver’s License issued by the Land
Transportation Office (LTO) of General Santos City;
2. That on March 14, 2018 the above mentioned driver’s license was lost
and could no longer be found despite diligent efforts to locate the same;
3. That my said driver’s license was not revoked , confiscated, or being
withheld by lawful authorities for violation of any law, rules or
regulations;
4. That I am executing this affidavit to attest to the truth of the foregoing
facts and for whatever purpose this may legally serve.

1. That I am a holder of Plane Ticket route: Manila-Kuala Lumpur-Manila,
which I purchased through Marazul Travel, Inc.-Marisa Bautista;
2. That the just recently, when I looked over for said plane ticket, the same could
not be found;
3. That diligent efforts were exerted to locate my said plane ticket, proved futile
as the same could no longer be found or located;
4. That I am executing this affidavit to attest to the truth of the foregoing facts
and for whatever purpose this may legally serve.
