This Make-up Artist Contract (the "Agreement"), executed on __________________, is entered
into by and between: Trixia Faye P. Catalan, with address in Olongapo City, (hereinafter
known as "Artist"); and __________________________ with address
in _______________________ (hereinafter known as "Client" and both collectively known as
"Parties");
WHEREAS, Client desires to hire Artist for performing make-up services for client;
WHEREAS, Artist accepts to engage to perform make-up services (the "Service") to Client for
the amount of __________________;
NOW, WHEREFORE, FOR and in consideration of the foregoing premises, the Parties hereby
agree as follows:
Time and Place
The Service shall be conducted beginning __________________ and shall end
at __________________ (the "Event") at the following address: _________________.
Terms of Payment
Client shall pay __________________ of the total contract amount and shall pay the remaining
balance after completion of the Service.
Client shall pay 50% of the fee as a deposit to confirm the booking of the date, time and the
location of the event.
The remaining amount will be due within 3 days of, or on/before the day of the event.
Cancellation
In case Client cancels this Agreement ____________ week prior to the scheduled event, all
deposits made by Client to Artist shall be non-refundable. In case Client cancels this Agreement
with more than __________week from the scheduled event, a cancellation fee of
_______________ shall be collected by Artist.

Expenses Reimbursement
Client hereby agrees to reimburse the Makeup Artist for any and all reasonable expenses
incurred as a result of performing the services detailed in this contract. Such expenses will
include but not be limited to travel, hotel stay, parking expenses, and/or cost of any materials or
supplies necessary for the Makeup Artist to perform their services for that specific event.
Client agrees to reimburse the Makeup Artist within _____ days of receiving an itemized
invoice from the Makeup Artist.
Responsibilities
Makeup Artist responsibilities:
Makeup Artist shall be responsible for providing their own makeup and equipment.
Furthermore, the Makeup Artist will provide their own transportation to and from the event.
Client responsibilities:
Client shall be responsible for providing the Makeup Artist with a detailed description of the
look they desire. Furthermore, the Client must supply the Makeup Artist with a list of any
allergies or sensitivities they have.
Social Media Rights
The Makeup Artist has the right to post photographs and videos of the Client on social media
platforms to promote their work, understanding that the Client's identity shall be held
confidential.
By signing below, he Client and Makeup Artist agree to the terms of this contract.
