Dear Parents and Guardians,
Greetings in St. La Salle!!
The NSTP and Formation Office (NFO) is pleased to inform you that your child/ward has opted to take the Civic Welfare Training
Service (CWTS) or the Literacy Training Service (LTS) component under the National Service Training Program (NSTP). This
letter hopes to give you an overview of the off-campus activity your child/ward may undergo.
Since the academic year 2020-2021, the NSTP CWTS and LTS components have been conducted in a full online mode. This type
of learning instruction prevented opportunities for face-to-face interactions among students and their partner organizations.
This term, as restrictions and government’s health and safety protocols eases, the NSTP and Formation Office (NFO) would like
to provide an opportunity for students and partner organizations to meet at their designated community to turn over project outputs,
conduct a project evaluation, and have their closing activity to conclude their NSTP CWTS/LTS journey.
The off-campus activity is an optional activity. Despite the easing of restrictions, the NFO acknowledges health and safety
concerns of its program stakeholders.
Rest assured that non-participation to the activity will have no impact on the attendance and participation grade of your child/ward
in the course.
The following will also be required from the students in order to participate in the said off-campus activity:
1. Medical Clearance from the University
2. Notarized Official Letter of Permission
For both requirements, costs for the Physical Examination and Notarization of the Letter of Permission will be shouldered by the
students.
Schedule for the off-campus activity are as follows:
November 26, 2022 for NSTPCW2 and NSTPLT2 Morning Sections only
December 3, 2022 for NSTPCW2 and NSTPLT2 Afternoon Sections only
Transportation through the use of DOT-accredited tourist buses will be provided to bring and return the students from DLSU to
the community and back. Their assigned Social Engagement Lecturer and a DLSU Security Officer will accompany them during
the said activity.

Not Allow my child/ward to join the scheduled Off-Campus Activity
I am also informed that the costs for Physical Exam and Notarization of Letter of Permission will be shouldered by the student. I
also understand that the information I am about to give shall only be stored and accessed by NFO CWTS/LTS Staff and will only
be used for DLSU NSTP CWTS/LTS program administration.
Deadline of the submission of the conforme slip is on or before October 15, 2022.
