PETITION
Petitioner, through the undersigned Senior Agrarian Reform Program Officer
(SARPO) and unto this Honorable Adjudication Board, most respectfully states that:
1. Petitioner is Filipino, of legal age, married to Pedro N. Perolina and a resident
of Tanayan, Roxas, Zamboanga del Norte where she may be served with summons and
other processes from this Honorable Board;
2. Respondent is the Register of Deeds of the province of Zamboanga del Norte
who is in charge in the issuance of certificates of title and with office address at
Provincial Capitol Complex, Dipolog City;
3. The petitioner is applying for the rights and privileges of a pauper litigant as
provided for in Section 23, Rule V of the 2021 DARAB Revised Rules of Procedure;
4. Rosita G. Ferolino is the registered owner of a certain parcel of agricultural
land known as Lot No. 1974-J, Psd-09-041044-AR/VLT covered by Transfer Certificate
of Title No. T-15263, situated in Tanayan, Roxas, Zamboanga del Norte with an area of
TWELVE THOUSAND NINE HUNDRED NINETY-EIGHT (12, 998) SQUARE METERS.
Machine copy of TCT No. T-15263 is hereto attached and marked as Annex “A” hereof;
5. The above-described agricultural land was placed under the coverage of
Republic Act (RA) 6657 as amended by RA 9700;
6. In the certificate of title, the family name of the ARB was erroneously written
as Ferolino instead of Perolina. Machine copy of the certification as to marriage of Pedro
N. Perolina and Rosita B. Galleposo is hereto attached as Annex “B”;
7. Such fact was also confirmed in a Joint Affidavit of Two Disinterested Persons
executed by Michael Tan and Maria Cielo Robles marked as Annex “C” hereof;
8. There is a need to change this entry to prevent future complications.
PRAYER
WHEREFORE, premises considered, it is most respectfully prayed of this
Honorable Adjudication Board that an Order be issued directing the Register of Deeds
of the Province of Zamboanga del Norte to change the family name of the ARB from
FEROLINO to PEROLINA.

correct as of my own knowledge and/or based on the records on hand;
I attest to the authenticity of the annexes thereof;
I further certify that I have not commenced and/or am not aware of any other
action or proceeding involving the same land, or a portion thereof, or issue in any court,
tribunal or quasi-judicial agency; and to the best of my knowledge, no such action or
proceeding is pending in any court, tribunal, or quasi-judicial agency, and if I should
thereafter learn that the same or similar action or claim has been filed or pending, I
shall report that fact within five (5) days from such notice.
