AFFIDAVIT OF LOSS

I, ____________________________, of legal age, Filipino citizen, and a resident


of ____________________________, Philippines, , after having been duly sworn to in
accordance with law, hereby depose and say that:

1. I am a holder/owner of a certain passbook bearing Account No.


111055090859 duly issued by 1st VALLEY BANK- _________ Branch,
______________________________
2. Said Passbook was usually kept inside the cabinet at our residence for
safekeeping.
3. However, due to inadvertence and/or oversight, I discovered that my Passbook
was no longer in the place where I kept it and its whereabouts is nowhere to be
found;
4. I exerted diligent efforts to look and locate for said Passbook in other places,
but all such efforts proved futile and to no avail;

5. For all intents and purposes, I am now declaring the above-mentioned


Passbook lost and beyond recovery;

6. I execute this Affidavit to attest to the truth of the foregoing facts based on
personal knowledge and for whatever legal purpose it may serve.
