Diagnosis: Left-side ischemic stroke, MCA (middle cerebral artery)
Ongoing hypertension (BP 175/100 mm kg)
Dyslipidemia

Medical history 2017 - Left-side ischemic stroke, MCA territory


2016 - Dyslipidemia (LDL: 180 mg/dL)
20112- Diagnosed with hypertension - Enalapril
2003 - Vitreous detachment in left eye
2001 - Surgery on ankle fracture
No allergies
Never smoked
Family history of diabetes
Medications:
Treated immediately on admission with TPA (tissue plasminogen activator) and
supplemental oxygen
15 May: commenced on dipyridamole (200 mg b.d)
Aspirin (75mg o.d)
Enalapril (25 mg o.d)
Metoprolol (50 mg b.d)
Atorvastatin (40 mg b.d / at night): LDL goal 70mg/dl
Anti-coagulant (Dabigatran: 75 mg b.d)

Medical progress:
Remaining aphasia and apraxia
Upper extremely weakness
Some muscle weakness in right arm (patient left-handed)
Some difficulty in dressing, showering & toileting due to upper extremity
weakness
Current BMI = 30 (gradual weight loss of 3kg)

Nursing management:
Regular physiotherapy – stretching & strengthening exercises
Screening for swallowing deficits – non observed
Nutrition assessment: Mediterranean – type diet recommended
Language therapy for aphasia and apraxia

Assessment: Good physical progress overall


Neurology consultation due: 22 may

Discharge plan: Monitor medications


Monitor BP (goal <145/90 mmHg)
Screen for diabetes
Dietician advice: reduce BMI
Lower cholesterol (goal LDL <70 mg/dl)
Continue physio & language therapy
Screen for depression
Encourage leisure & social activities

Writing Task:
