The nursing assessment identified a pregnant patient with no complaints who eats fruits and vegetables and sleeps 8-10 hours per night. Laboratory tests were normal. The nursing diagnosis identified a knowledge deficit about pregnancy and childbirth. Short term goals included educating the patient on vital signs and prenatal health. Long term goals were to encourage a balanced diet, exercise, rest and education on prenatal care, labor signs, coping and postpartum care. The plan included collaborating with a dietitian, monitoring weight gain and fetal growth through appointments and ultrasounds, and creating a supportive environment for the patient to discuss concerns.
