AFFIDAVIT OF UNDERTAKING

I, Veronica Centeno Beltran, widow, Filipino, of legal age and with


residence at # 22 Gannaban St. Centro Norte, Alcala, Cagayan, after having
been duly sworn to in accordance with law hereby depose and state:

1. That I am the wife of the late Luis A. Beltran who died on April 7
2023;

2. That my husband was a bonafide member of the Armed Forces and


Police Savings & Loan Association, Inc. Philippine Army Finance Center
Producers Integrated Cooperative (PAFCPIC);

3. That consequently, I am requesting PAFCPIC authorities to allow me


to receive the proceeds of the termination of the deceased's account for my
benefit and also for the benefit of other heirs;

4. That I will hold myself criminally and civilly responsible for any
liability that may arise in connection with my receipt of the above-cited
proceeds or benefit and further free PAFCPIC from liability relative thereto.
