Sub: Return of Metro Smart Card and approval for refund of security deposit and card
balance

Dear Sir,

This is to inform you that two of my Smart Cards are not functioning. The card numbers are as
follows:

1. Metro Card No 1: 010184502 (Metro Card Copy Enclosed)


2. Metro Card No 2: 011076382 (Metro Card Copy Enclosed)

I hereby request you to kindly accept the return of the cards and refund the security deposit along
with the remaining balance of the cards.
