1. Reference: Audit Observation Memorandum (AOM) no. 2023-001 dated August 07, 2023

2. In connection with above reference, please be informed that the undersigned could not
renew the fidelity bond as a collecting agent on time because of failure to notice the schedule
for renewal..

3. Moreover, please be informed that the undersigned already renewed the fidelity bond and
will make sure that this instance will never happen again. Hence, the undersigned has no
intention of neglecting his tasks as collecting agent and therefore seek before your good office
for consideration thereof.

4. Request acknowledge receipt.
